Who Actually Administers Your Cigna Behavioral Health Benefit

Who Actually Administers Your Cigna Behavioral Health Benefit

Cigna administers its own behavioral health benefits through Evernorth, its in-house behavioral arm, so for most Cigna members the authorization and network questions go to Evernorth rather than to Cigna’s medical side. There is one important exception in Tennessee, and it applies to a large group of people.

If your Cigna card comes from the State of Tennessee Partners for Health plan, your behavioral health is not administered by Evernorth at all. That plan carves substance use and mental health benefits out from the medical carrier and assigns them to Optum, under the Here4TN program. Your medical claims go to Cigna and your treatment claims go to Optum.

This affects anyone on that plan, including state employees, higher education staff, local education and local government employees, their dependents, Partners for Health retirees and COBRA members. It changes who reviews your prior authorization, which network rules apply and which phone number gets you a real answer.

If you are not certain which applies to you, call (615) 645-3677 with your card in hand. Our admissions team will identify the administrator first, then verify your benefits with whichever one actually holds them.

What Levels of Care Does Cigna Cover at Freeman Recovery Center?

Cigna behavioral health benefits typically extend across the full continuum of addiction treatment, from medically monitored detox through outpatient maintenance. Below is a breakdown of the levels of care Freeman Recovery Center offers and how Cigna coverage generally applies to each.

Medical Detox

Medical Detox

Medically monitored detox at FRC’s Burns, TN campus operates 24 hours a day, 7 days a week, with clinical oversight throughout withdrawal. For substances like alcohol, benzodiazepines, and opioids, medical detox is often considered medically necessary, which means Cigna plans frequently cover detox when clinical criteria are met. Prior authorization is usually required before admission or within the first 24 hours, and our admissions team handles this process on your behalf.

Residential Treatment

Residential Treatment

Inpatient residential care at the Burns campus includes 24/7 structured programming, individual therapy, group sessions, and trauma-informed counseling. Cigna covers residential treatment when clinical criteria for medical necessity are satisfied, with coverage lengths depending on your specific plan and ongoing utilization reviews. Freeman Recovery Center offers short-term residential options (7, 14, 21, or 30 days) as well as longer-term programs (60, 90, or 180 days) to match both clinical need and what your benefits support.

Partial Hospitalization Program (PHP)

Partial Hospitalization Program (PHP)

PHP at FRC’s Dickson outpatient location provides daytime clinical programming while you live at home or in supportive recovery housing. PHP is frequently covered by Cigna as a step-down from residential or as a starting point for those who do not require inpatient placement. FRC also offers PHP through Freeman Recovery Online, the telehealth program, for patients across Tennessee who have transportation barriers or work obligations.

Intensive Outpatient Program (IOP)

Intensive Outpatient Program (IOP)

IOP is one of the most commonly Cigna-covered levels of outpatient addiction treatment. Freeman Recovery Center’s IOP includes several hours of structured therapy per week, with evening scheduling available for working adults, parents, and those with family responsibilities. Cigna-covered IOP is available in person at the Dickson facility and virtually through Freeman Recovery Online.

Standard Outpatient and MAT

Standard Outpatient and MAT

Ongoing outpatient care and Medication-Assisted Treatment (MAT) using FDA-approved medications such as buprenorphine, naltrexone, and acamprosate are available at FRC and through Freeman Health Partners, FRC’s integrated sister organization. Cigna plans often cover MAT combined with counseling, as this approach aligns with SAMHSA evidence-based treatment guidelines. Cigna coverage for MAT medications may be governed by your pharmacy benefits, which our team can help you navigate.

Dual Diagnosis Treatment

Dual Diagnosis Treatment

Many people seeking addiction treatment are also managing a co-occurring condition such as depression, anxiety, PTSD, or a mood disorder. Cigna’s behavioral health parity requirements mean that dual diagnosis treatment, which addresses substance use and mental health simultaneously, should be covered at the same level as either condition treated independently. FRC integrates Cigna-covered dual diagnosis care across all levels, from detox and residential through PHP and IOP.

How to Verify Your Cigna Rehab Benefits Before Admission at Freeman

The fastest way to understand what your Cigna plan covers at Freeman Recovery Center is to let our admissions team run a free verification of benefits (VOB) for you. Here is what that process looks like:

    1. Call (615) 645-3677 or submit your insurance information online
    2. A member of our admissions team contacts Cigna directly to confirm your active coverage, applicable deductible, copay or coinsurance amounts, and any prior authorization requirements for the level of care you need
    3. You receive a clear picture of your estimated out-of-pocket treatment costs before admission
    4. If same-day admission is appropriate and clinically indicated, we can move forward immediately

Same-day admissions are available at Freeman Recovery Center. You do not need to wait days to get answers about your coverage or to enter care.

What Cigna Typically Requires for Addiction Treatment Authorization

Understanding Cigna’s standard authorization process helps set realistic expectations. Most Cigna behavioral health plans require the following for addiction treatment:

    • Prior authorization for residential treatment, PHP, and in some cases IOP, before or shortly after admission
    • Medical necessity documentation from a licensed clinician confirming the appropriate level of care
    • Utilization reviews during residential stays, where a Cigna case manager periodically reviews clinical records to confirm ongoing medical necessity for the current level of care
    • Step-down criteria that guide when a patient transitions from residential to PHP, IOP, or outpatient

Freeman Recovery Center’s clinical team has extensive experience managing Cigna utilization reviews and prior authorization submissions. This is handled by our staff, not by you.

What You Might Pay Out of Pocket with Cigna at Freeman Recovery

Exact out-of-pocket rehab costs depend on your specific Cigna plan, your deductible status, and whether Freeman Recovery Center participates as in-network for your plan type. The main cost variables to understand are:

    • Deductible: The amount you pay before insurance begins covering costs. If you have already met your annual deductible, your cost share for treatment may be significantly lower.
    • Copay or Coinsurance: A fixed fee or percentage of covered charges you pay per day or per service. Cigna plans vary widely here.
    • Out-of-Pocket Maximum: Once you reach this annual limit, Cigna covers 100% of covered services for the remainder of the year. A residential or PHP stay can move you toward this threshold quickly.

For a rough cost reference, a 30-day stay at Freeman Recovery Center runs approximately $18,000 before insurance. What you actually pay depends on your Cigna benefits. Many patients find that insurance covers the majority of treatment costs. For a specific estimate tied to your plan, the only way to know for certain is to complete a free benefits verification.

Other Ways to Cover Rehab Costs Alongside Cigna

When insurance covers only a portion of treatment, or when there is a gap between what Cigna pays and the total cost, Freeman Recovery Center offers additional financial options:

    • Sliding-scale payment based on income and family size through Freeman Health Partners
    • State-funded treatment options through TennCare/Medicaid for those who qualify
Payment plans to spread remaining costs over time
    • Transparent cost guides available on the FRC website for each treatment category

The goal is to make sure cost is not the reason someone does not access treatment. Our admissions team can walk through all available options with you confidentially.

Frequently Asked Questions: Cigna Coverage for Addiction Treatment

My Cigna card is a State of Tennessee plan. Does Evernorth handle my rehab benefits?

My Cigna card is a State of Tennessee plan. Does Evernorth handle my rehab benefits?

No. The State of Tennessee Partners for Health plan carves behavioral health out to Optum under the Here4TN program, even though your medical card says Cigna. Prior authorization, network status and level-of-care decisions for addiction treatment run through Optum, not Evernorth. Call (615) 645-3677 and we will confirm which administrator holds your benefit before we verify anything.

What is Evernorth and why does my rehab claim go there?

What is Evernorth and why does my rehab claim go there?

Evernorth is Cigna’s own behavioral health organization. For most commercial Cigna plans it is the entity that reviews medical necessity, issues prior authorizations and maintains the behavioral health network, which is separate from the medical network on the front of your card. A facility can be in network on one side and not the other, which is why the verification checks the behavioral side specifically.

What happens if Cigna denies authorization for the level of care my clinician recommends?

What happens if Cigna denies authorization for the level of care my clinician recommends?

A denial is not the end of the process. Our clinical team can request a peer-to-peer review, where the treating clinician speaks directly with the reviewing physician, and can file an appeal with additional clinical documentation. You also have the right to an independent external review if the internal appeal is denied. Our staff handles the submissions rather than leaving them to you or your family.

Does Cigna cover treatment if Freeman Recovery Center is out of network for my plan?

Does Cigna cover treatment if Freeman Recovery Center is out of network for my plan?

Often, yes, at higher cost sharing. Many Cigna plans include out-of-network behavioral health benefits, and some plans grant a single case agreement when an in-network facility with the right level of care is not available within a reasonable distance. Both are worth checking before you rule out treatment on cost. We check both during verification.

Does my employer’s self-funded plan follow the same parity rules?

Does my employer’s self-funded plan follow the same parity rules?

Federal parity under MHPAEA applies to most employer group health plans, including self-funded ones, but self-funded plans are exempt from state insurance mandates and administer their own benefit designs. In practice that means the protections are similar and the specific limits, authorization rules and networks can differ noticeably from a fully insured Cigna plan. The verification is what settles it.

How long does prior authorization take, and can I start before it comes through?

How long does prior authorization take, and can I start before it comes through?

For detox, authorization is usually initiated before admission or within the first 24 hours, because withdrawal is time sensitive. Residential and PHP typically require authorization before admission. Same-day admission is possible when coverage and clinical need are confirmed quickly. Our admissions team tells you which timeline applies to your situation on the first call rather than after you arrive.

I came in and instantly wanted to leave I was scared and assumed how this place was based on looks then I made it to class and met the people and employees and fell in love! Like anywhere else its what you make of it. The EMDR therapy was phenomenal I was not a believer but it was one of the most helpful things I did I also enjoyed the Acupuncture. The only complaint I have is the staff that I loved are no longer there. Please reconsider bringing them back they helped me see the light!

This is my second review. It will be just wonderful as my first. My husband was here last year and they were wonderful. Unfortunately at no fault of anyone there he is now back. This second time they were just as wonderful. They got him a bed the first day he called them. When he arrived they all welcomed him again. No one ever wants to have their family member to have to go through addiction but thank God for people like Freeman who truly care and want nothing but the best. I look forward to watching my husband’s progress and being there every step of the way. Thank you Freeman for caring and not ever giving up.
